What Are Housing Stabilization Services?
Housing Stabilization Services is a Medical Assistance program designed to support individuals with disabilities, including those dealing with mental illness and substance use disorders, as well as seniors, in their quest to secure and sustain housing. Securing and maintaining stable, affordable housing is pivotal to enhancing the overall health and well-being of these individuals.
The purpose of these services is to facilitate an individual’s transition into housing while fostering long-term stability within the community, preventing future instances of homelessness or institutionalization.
Am I Eligible For Housing Stabilization Services?
You may be eligible for Housing Stabilization Services if you meet the following criteria:
- Having a Disability: You are a person with a disability.
- Medical Assistance & 18+: You have Medical Assistance and are 18 years of age or older.
- Experiencing Housing Instability: You are facing housing instability.
- Desire for More Independence: You are in a living situation that is currently provider-led and wish to transition into a more independent setting.
- Adults with Applicable Waivers: You are an adult with an applicable waiver such as CADI, DD, BI, EW, or CAC waiver.

- Coordinating Moving

- Keeping the Client Housed
- Educating and Coaching to Develop and Maintain Key Relationships with Property Managers and Neighbors
- Educating on Roles, Rights and Responsibilities of a Tenant
- Supporting the Person in Applying for Housing Benefits and Resources
- Arranging for Adaptive House-Related Accommodations
